National Wheaten Rescue Forms and Applications

Click on the links below to access some of NWR’s forms and applications.  Guidance about when each form is requested by NWR is available on the applicable pages throughout our website.  Other forms or information, if/when required, will be provided by one of NWR’s Rescue Specialists.

How to Complete and Submit our Forms

A network of volunteers and adopters all across the United States make our mission possible! (Logistics and other considerations prevent us from doing our work in Alaska, Hawaii, Canada or Mexico).  The Volunteer Driver Application, the Adoption Application, and the Foster Home Application are Online Submission forms.  After you click on the link that opens the form, complete it then simply click “submit” at the end of the form.

Our fillable forms are designed so they can conveniently be completed right on your computer then emailed to us.  Here’s how to submit fillable forms:

Step 1:   After you click on the link that opens the form, save it on your computer by downloading it.
Step 2:   Open the newly saved document to add your answers/information directly in the available fields (text boxes or check boxes, as may be applicable) then resave the form.  Most people resave the completed version with a file name that includes their name at the end of the form name.
Step 3:   Email the completed form to rescue@nationalwheatenrescueinc.org.

If the downloaded form doesn’t allow you to type in the fillable fields, your browser or software may not support that feature.  You may print the form, complete it, then scan it for email back to rescue@nationalwheatenrescueinc.org.

  • Adoption Application – Online Submission

    For more information about to adopt a Wheaten from National Wheaten Rescue, refer to our ADOPT page.  Applications are accepted from US residents all across the country (every state except Alaska and Hawaii) and are considered active for a full year from date of completion.  This form is automatically entered into our application database when you click on “submit” at the end of the form after you complete it.

  • Adoption Agreement – Available for review prior to adopting a Wheaten

    This form is sent to Adopters at the time an individual dog is being offered for adoption.  It will be completed and sent to the Adopters by one of NWR’s Rescue Specialists, and will include the details and information specific to that individual dog.  For more information about the adoption steps and process, refer to our ADOPT page.

  • Foster Home Application – Online Submission

    For more information about applying to foster a rescued Wheaten, refer to the FOSTER section of the SUPPORT NWR page.  This form is automatically entered into our application database when you click on “submit” at the end of the form after you complete it.

  • Foster Home Agreement – Fillable form for emailing to us

  • Wheaten Ownership Transfer Agreement – Fillable form for emailing to us

  • Volunteer Driver Application – Online Submission

  • Volunteer Driver Agreement – Fillable form for emailing to us

    For help with how to complete this form, refer to the section above about how to complete and submit our fillable forms.  This form is signed and submitted by every type of volunteer involved in NWR rescue interventions and by all parties involved in surrendering, fostering, or adopting a dog.
